Is Home Health right for my loved one or me?

See below for a list to consider. Keep up with how many you answer yes.


  • Have you been diagnosed with a new illness?

  • Have you had a recent hospitalization?

  • Has your medication recently changed?

  • Do you require skilled nursing services?

  • Do you require therapy services?

  • Would you rather be cared for at home?

  • Are you losing your balance and falling?

  • Have you had surgery or an injury recently?

  • Do you have wounds that are not healing?

When is it the right time for Hospice?

See the list below to learn some of the signs that it might be time for hospice. Preferred Hospice is provided to clients under the certification and direction of a licensed physician. Anyone can refer to hospice. We are happy to answer any questions regarding eligibility, services provided, or potential needs.


  • Frequent visits to the ER or hospital admissions

  • A decline in their ability to perform daily tasks including eating, getting dressed, walking, or using the bathroom

  • An increase in falls

  • Changes to their mental abilities

  • Progressive weight loss

  • Skin tears, infections, and other signs of deteriorating health
